The Florida Uniform Mitigation Verification Inspection Form, Revision 04/26 — adopted by Rule 69O-170.0155, F.A.C. — is what determines a homeowner's wind-mitigation insurance discount. Here's exactly what every section asks, what evidence the inspector needs, and how to read the form when it comes back.

OIR-B1-1802 — Quick Reference

Where is the official form?

Florida Office of Insurance Regulation publishes it at floir.gov. The current revision is 04/26, adopted by Rule 69O-170.0155, F.A.C. InspectorData ships the current version inside the platform.

How long is it?

Six pages. Owner information block, nine numbered questions, an inspector certification block, and a homeowner attestation.

What's required as proof?

At least one photograph or document for each attribute marked in questions 2 through 9. FORTIFIED Home certificates can substitute for some attributes in questions 4-6 and 8-9.

Who must sign?

A qualified inspector per Section 627.711(2), F.S. Home inspectors with hurricane mitigation training, building code inspectors, contractors, engineers, architects, or others recognized by the insurer.

The Owner Information Block

The first page captures basic property and policy information. None of this affects the discount — but every field must be filled or the carrier will return the form.

  • Inspection date
  • Owner name + contact person
  • Address, City, Zip, County
  • Phone numbers (home, work, cell)
  • Insurance company + policy #
  • Year of home + number of stories + email

Question 1 — Building Code

Three options based on which Florida code was in force when the original permit was issued, plus an "unknown" option.

  • A. FBC 2001 & 2004 — for homes built 2002/2003, requires permit application date after 3/1/2002
  • B. FBC 2007 and later — for homes built 2007/2008, requires permit application date after 12/8/2006
  • C. SFBC-94 (HVHZ Only) — Miami-Dade or Broward, year built 1994-1996, permit application after 9/1/1994
  • D. Unknown — or doesn't meet A/B/C requirements

Question 2 — Region

Location-based wind-speed classification. References ASCE 7-22 (700-year MRI) Risk Category 2:

  • HVHZ — Miami-Dade and Broward counties
  • Region 1 — design wind speed ≥ 140 mph
  • Region 2 — 130-139 mph
  • Region 3 — < 130 mph

Question 5 — Roof Deck Attachment (the Discount Driver)

The single most consequential section for the discount. Documents the WEAKEST roof deck attachment present anywhere on the roof.

  • A. 6d nails or staples at 6"/12" — minimum (55+ psf uplift)
  • B. 8d nails at 12" — middle tier (103+ psf uplift)
  • C. 8d nails at 6" — strongest typical wood deck (182+ psf uplift)
  • D. Reinforced concrete deck — strongest of all
  • E. Spray foam (110 psf with FOS 1.5)
  • F. Other / G. Unknown / H. No attic access

Going from A to C can move a homeowner's premium meaningfully. Photo evidence: nail pattern visible in attic with a tape measure for scale.

Question 6 — Roof-to-Wall Attachment

The other major discount driver. Five named tiers plus other/unknown:

  • A. Toenails — bottom tier (185 lbs uplift documented)
  • B. Clips — non-wrap connectors or 1-strap connectors not meeting C/D (386 lbs)
  • C. Single Wraps — 1 strap wrapping over the truss (535 lbs)
  • D. Double Wraps — 2 separate straps OR 1 strap with 3+ nails each side (891 lbs)
  • E. Structural — anchor bolts or reinforced concrete
  • F-I. Other, unknown, no attic, or not installed as intended

Note: Hip/valley jacks within 5 feet of inside/outside corners are excluded from the WEAKEST determination.

Question 9 — Opening Protection (the Trickiest Section)

Asks the WEAKEST form of wind-borne debris protection across all openings (windows, doors, sliding glass, skylights, garage doors). The Opening Protection Level Chart classifies each opening type with a level (A through Z):

  • A. Cyclic pressure + 9-lb large missile (4.5 lb skylights) — Level A
  • B. Cyclic pressure + 4-8 lb large missile
  • C. Verified plywood/OSB meeting FBC 2007 Table 1609.1.2
  • D. Non-glazed entry/garage doors compliant with ASTM E 330, ANSI/DASMA 108, or PA/TAS 202
  • N. Unverified — products that appear to meet A or B without documentation
  • X. No protection at all
  • Z. Damaged openings needing repair/replacement

The form has sub-options (A.1, A.2, A.3, B.1, etc.) that further classify based on non-glazed opening levels. Read carefully — this section is where most Wind Mits are filled out incorrectly.

The Inspector Certification Block

Page 5 includes the qualified inspector certification. The inspector checks ONE box for their license type and signs. The signature attests they personally performed the inspection (or, for licensed contractors and PE engineers, that an authorized employee did under their direct supervision).

Page 6 has a homeowner attestation: the homeowner signs confirming the inspector (or employee) physically performed the inspection and showed identification.

Important: Filing a fraudulent Wind Mit is a first-degree misdemeanor under Section 627.711(7), F.S. The form means what it says.

Frequently Asked Questions

What if I find a feature is borderline between two tiers?

Document carefully and choose the WEAKEST per the form's instruction. The form requires the WEAKEST present anywhere on the structure, not the strongest. Carriers will sometimes downgrade your answer if photos suggest a different classification.

Can I use a FORTIFIED Home certificate instead of photos?

Yes — for questions 4-6 and 8-9 only. Check the certificate type (Roof, Silver, or Gold) and attach a photocopy. Questions 1-3 and 7 still need their own documentation.

What does 'No attic access' mean for the discount?

If you can't get into the attic, you can't verify roof deck attachment (Q5) or roof-to-wall attachment (Q6). The carrier won't grant the discount for those features. Some homeowners install a scuttle access and get a re-inspection.

Why does the form take photos so seriously?

Carrier underwriting fraud is a real problem in Florida. Mitigation fraud is a first-degree misdemeanor and the form explicitly references the Bureau of Insurance Fraud. Photo documentation protects everyone — homeowner, inspector, and carrier.

What's the difference between revisions of the form?

OIR has issued multiple revisions over the years. The current adopted version is 04/26 (Revision date April 2026). When OIR publishes a new revision, all carriers must accept it — and inspectors should switch immediately. Older revisions may not be accepted.

Can I write notes outside the form fields?

No. The form is the form. Inspector observations beyond the structured fields go in a separate report. The OIR-B1-1802 itself is structured exactly as adopted by rule and must be filled as designed.
